How should you preregister for the COVID Vaccine?

 Complete this FORM and contact your local clinic, pharmacy, primary care doctor, or a federally qualified health center near you to see if they are taking appointments.

As of Friday, March 19, 2021......

The City of Philadelphia is still in 1A and 1B.   Generally speaking, this means you need to be over the age of 65 to schedule an appointment for your vaccine.

To get an appointment at Cannstatter, you need to be registered with the City of Philadelphia.  When in doubt, register with the City again.  This site is being run by the City of Philadelphia and will be staffed by the Philadelphia Fire Department. Invitations to schedule are likely to be sent out this weekend, so check your email regularly.

If you are under the age of 65 and eager to get an appointment, you may qualify to walk up at the FEMA site downtown at the convention center.  Things to know. You MUST be a 1A or 1B.  You must reside in one of the targeted zip codes. If you are a crossing guard, teacher.... CHOP is your vaccine provider and you should get scheduling information from the principal you are working with/for. If you are having a hard time getting an appointment, let us know.
